Camper Activity Form
(Not required for Youth Camp Aug 22)
You get to choose your activities while you're at camp!!!
Simply fill out your name, email and date that you are attending camp and then select the activities you would like in order of preference. A confirmation email will be sent to you saying that your information was recieved.
If you have registered for more than one week and wish to select different activities for your second week then please complete a second form.
